Before August 1 of a school year for which a school is placed in academic receivership, the superintendent and the board shall require the following:

1. (1) Evaluation of each administrator at the school.
2. (2) Evaluation of each teacher at the school.
3. (3) Evaluation of the school's educational plan.

4. (4) Consideration of:

   1. (A) personnel;
   2. (B) management;
   3. (C) plan; and

   4. (D) policy;

      changes for the school to improve student performance at the school.

5. (5) Identification of significant;

   1. (A) management;
   2. (B) personnel;
   3. (C) plan; or

   4. (D) policy;

      changes that in the board's judgment must be implemented to improve the school's performance.
