1. (a) The statewide maternal mortality review coordinator shall develop a confidentiality form for use by the statewide maternal mortality review committee and any individuals who are invited by the chairperson to attend a meeting of the statewide maternal mortality review committee.
2. (b) Any individual who is invited by the chairperson to attend a meeting of the statewide maternal mortality review committee shall sign the confidentiality form described in subsection (a) in order to attend the meeting.
