An individual is eligible to participate in a program if the individual:

1. (1) is an Indiana resident;
2. (2) has an income that does not exceed seven hundred percent (700%) of the federal poverty level;
3. (3) resides in an underserved area, as determined by the organization or entity; and
4. (4) is eligible to participate in a cancer clinical trial.
