1. (a) As used in this chapter, "diagnostic imaging facility" means an entity that:

   1. (1) provides diagnostic imaging to an individual for the purpose of health care; and
   2. (2) is not owned or operated by a hospital licensed under IC 16-21-2 .

2. (b) The term does not include the following offices where an x-ray or imaging service is exclusively used by the office:

   1. (1) A dental office.
   2. (2) An optometrist office.
   3. (3) A practitioner's office.
