Ind. Code § 16-41-14-12

Donor information

As added by P.L.2-1993, SEC.24.

  1. (a) A practitioner shall require a semen donor to provide the following information:

    1. (1) Name.
    2. (2) Address.
    3. (3) Date of birth.

  2. (b) A practitioner shall request a semen donor to provide the semen donor's Social Security number.
