If a certificate of birth is issued from the record described in section 16 of this chapter, the certificate:

1. (1) must include:

   1. (A) the full name of the child;
   2. (B) the date and place of birth;
   3. (C) the name of the father;
   4. (D) the maiden name of the mother; and
   5. (E) other data required;
2. (2) must be uniform in color, size, and content with all other certificates; and

3. (3) may not refer to the fact that the child was born out of wedlock.
