If a paternity affidavit executed under section 2.1 of this chapter is filed with a local health officer, the local health officer shall do the following:

1. (1) Correct the local record of birth by adding the name of the father to the certificate of birth.

2. (2) File a copy of the affidavit with the report required by IC 16-37-1-6 .
