The storage facility must notify the state chemist each year of the facility's location and status. Notice must include the following:

1. (1) Facility's mailing address.
2. (2) Owner or manager.
3. (3) Type of facility.
4. (4) Rated or calculated capacity of all bulk tanks and dry storage units.

5. (5) Facility's physical location.

   Notice must be made upon forms furnished by the state chemist.
