1. (a) A request for a permit may be made by any person interested in the proposed work by filing with the department the following:

   1. (1) A brief statement and description of the work.
   2. (2) Plans and specifications for the work.
   3. (3) An investigation fee of a minimum of twenty-five dollars ($25).

2. (b) Subject to IC 14-10-2-1 , the commission may set an investigation fee above the minimum fee established under subsection (a)(3).
