1. (a) A person who knowingly or intentionally fails to comply with the requirements of:

   1. (1) a license issued under this chapter;
   2. (2) this chapter; or

   3. (3) rules adopted under this article to implement this chapter;

      is subject to suspension or revocation of the person's license.

2. (b) A license revoked under this section may not be reinstated.
3. (c) A person who knowingly or intentionally violates this chapter commits a Class A misdemeanor.
