Ind. Code § 12-25-1-5 – License application; additional information

An application must contain the following additional information:

(1) The name of the applicant.
(2) The type of institutions to be operated.
(3) The location of the institution.
(4) The name of the person to be in charge of the institution.
(5) Any other information the director requires.
