If supplemental assistance is granted to an aged person under this chapter, the division shall enter the following on a certificate:

1. (1) The award made.
2. (2) The name and residence of the recipient.
3. (3) The amount of the award.
4. (4) The date when the assistance to the recipient is to begin.

5. (5) Any other fact required by the division.
