A person shall be qualified for licensure as an occupational therapy assistant if that person:

1. (1) has applied in writing in form and substance to the Department;
2. (2)

4. (3) has completed an occupational therapy program of at least 2 years in length leading to an associate degree, or its equivalent, approved by the Department; and
5. (4) has successfully completed the examination authorized by the Department within the past 5 years.
