1. (a) That the individual is a resident of this State and a citizen or lawful permanent resident of the United States;
2. (b) That the individual has been accepted in a dental school located in Illinois;
3. (c) That the individual exhibits financial need as determined by the Department;
4. (d) That the individual has earned an educational diploma at an institution of education located in this State or has been a resident of the State for no less than 3 years prior to applying for the grant;
5. (e) That the individual is a member of a racial minority as defined in Section 3.07; and
6. (f) That the individual meets other qualifications which shall be established by the Department.
