1. a) Payment of the civil penalty shall be made by certified or cashier's check, by money order, or in installments by the foregoing means after execution of a promissory note containing an agreement for judgment.

2. b) All remittances shall be made payable to the Illinois Commerce Commission/TRF and sent to:
