Ill. Admin. Code tit. 92, § 1715.100 – Approval of Disclosure Forms

a) The Commission shall prescribe a uniform form for disclosures required by the Law.
b) A safety relocator may comply with disclosure requirements of the Law by:
1. 1) Using a form prescribed by the Staff of the Commission;
  1. 2) Using a form that contains all of the information contained in a form prescribed by the Staff of the Commission; or
2. 3) Using a form specifically approved by the Staff of the Commission.
