1. a) An applicant for an Identification Card age 16-20 who is a youth for whom the Department of Children and Family Services (DCFS) is legally responsible for or is a foster child and who submits a properly completed DCFS form number CFS 906/906-1 shall be issued an Identification card at no cost.
2. b) The CFS 906/906-1 form must be signed by a representative of DCFS and the applicant's caseworker and/or provider.
3. c) To be accepted, the CFS 906/906-1 form must contain the following information:
4. 1) Complete name and residential address of applicant;
5. 2) DCFS Client ID number;
6. 3) Provider name and address; and
7. 4) Provider signature and current date.
