By December 1, March 1, May 1 and August 1, the participant shall file, on a form prescribed by the Department, a statement of actual eligible expenses incurred in the 1^st, 2^nd, 3^rd and 4^th quarters respectively, known as the Quarterly Financial Report – Actual Revenues and Expenses.
