1. a) counseling and training for family caregivers and support service personnel in management of care;
2. b) functional and needs assessment services;
3. c) assistance with locating, arranging for, and coordinating services;
4. d) case management; and
5. e) counseling prior to admission to nursing home to prevent premature institutionalization.

In administering services under Title III-D, each area agency on aging shall establish and implement a documentable method to coordinate with other community agencies and voluntary organizations providing:
