1. a) In addition to any other Department record requirements, including those specified in 89 Ill. Adm. Code 140.28, a Children's Community-Based Health Care Center must maintain complete, comprehensive and accurate medical records to ensure adequate patient care that includes, but is not limited to, the following:
2. 1) Patient identification;
3. 2) Significant medical history;
4. 3) Any known allergies and abnormal drug reactions;
5. 4) Documentation of properly executed informed patient consent;
6. 5) Medications and treatments ordered and administered; and

7. 6) Clinical and nursing notes.

   1. b) Children's Community-Based Health Care Center records must contain the dates of service and the recorded dates of any medical practitioner seeing the patient at the time in the center.
   2. c) Medical records for Medicaid patients must be made available to the Department or its designated representative in the performance of any review by the Department.
   3. d) The Children's Community-Based Health Care Center agrees to furnish to the Department, if requested, information necessary to establish payment rates in the form and manner that the Department requires.
8. e) Services may be subject to on-site review by the Department.
