1. a) advise the client(s) that disclosure of or application for a Social Security Number is voluntary;
2. b) inform the client(s) by what statute the Social Security Number is requested; and
3. c) inform the client that Social Security Numbers will be used only in the administration of the Medicaid program.

The Department shall request the Social Security Number of each individual (including children) applying for assistance and;
