1. a) be a licensed child care provider holding a valid day care license from DCFS (see 89 Ill. Adm. Code 406, 407 or 408) or be a license-exempt center serving school age children and operating in accordance with DCFS exemptions (see 89 Ill. Adm. Code Part 377.3);

2. b) regularly provide year-round child care services;

   1. c) typically rely on private-pay tuition receipts and/or payments through DHS' Child Care Assistance Program (CCAP (see 89 Ill. Adm. Code 50)) for at least 25% of its revenues;

3. d) be open and receiving children at the time of application; and

   1. e) certify that its operating capacity has been reduced and/or costs have been increased because of COVID-19 restrictions.

To qualify for a Child Care Restoration Grant 2021, the applicant must be a child care provider that received a grant through the Department of Economic Opportunity Business Interruption Grant Program (20 ILCS 605/605-1050, 14 Ill. Adm. Code 690, Subpart C) or meet all five of the following requirements:
