1. a) For day care centers:

   1. 1) The number of eligible classrooms, as indicated on the program's application; and
   2. 2) The ages of the children enrolled in the eligible classrooms.
   3. b) For day care homes and group day care homes, the number of staff and hours worked.

Award amounts will be based on the following factors:
