- a) DHS-DRS will purchase a hearing aid evaluation for a customer who has a significant hearing loss, as documented by an audiometric test (89 Ill. Adm. Code 590.90).
b) A hearing aid evaluation shall consist of:
- 1) selection of an appropriate hearing aid, based upon the outcome of the audiometric test (89 Ill. Adm. Code 590.90);
- 2) fitting of the hearing aids and earmold impression, when appropriate;
- 3) adaptation and services of the hearing aids;
- 4) testing of the hearing aids by means of an audiometer calibrated to American National Standards Institute (ANSI) standards (77 Ill. Adm. Code 682.105(a)(1)); and
- 5) other testing allowed pursuant to 77 Ill. Adm. Code 682 and the Hearing Instrument Consumer Protection Act (225 ILCS 50).
