b) Information provided for each occurrence exceeding five days shall include:

1. 1) Date of interruption;
2. 2) Description of occurrence;
3. 3) Duration;
4. 4) Units affected;
5. 5) Inventory burned; and
6. 6) Size of inventory when deliveries resumed.
