1. a) the name and address of the petitioner;

2. b) the name and address of their authorized representative if applicable;

   1. c) a complete explanation of the factual and/or legal basis for the request, including relevant documentation supporting the petitioner's position;
   2. d) a detailed statement explaining how TRS has misapplied applicable provisions of the Code and/or administrative rules governing the System; and
   3. e) the relief sought by the petitioner.
