1. a) The Department requires the following health care facilities to report patient incident information regarding extensively drug-resistant organisms (XDROs):

   1. 1) Hospitals;
   2. 2) Hospital-affiliated clinical laboratories;
   3. 3) Independent or free-standing laboratories;
   4. 4) Long-term care facilities;
   5. 5) Long-term acute care hospitals (LTACHs);
   6. 6) Dialysis centers;
   7. 7) Specialized mental health rehabilitation facilities; and
   8. 8) Other high-risk health care facilities serving high-risk patients.

2. b) The Department requires the following health care facilities to query or implement alert notification with the XDRO Registry in order to identify new admissions with XDROs:

   1. 1) Hospitals;
   2. 2) Long-term acute care hospitals;
   3. 3) Skilled nursing and intermediate care facilities; and
   4. 4) Dialysis centers.
