1. a) A diabetes screening shall include an assessment of whether a child is overweight (see Section 665.720) and an assessment of the following risk factors:

2. 1) Family history of type 2 diabetes in first and second degree relatives;

   1. 2) Member of an ethnic minority group, including American Indians, African Americans, Hispanic Americans, Asian/South Pacific Islanders; and
   2. 3) Signs of insulin resistance or conditions associated with insulin resistance, including acanthosis nigricans, hypertension, dyslipidemia, or polycystic ovarian syndrome.
3. b) Results of the diabetes screening shall be documented on the Certificate of Child Health Examination form.
