Fee Assessment and Payment

Reg. 13014AUTHORITY: Implementing and authorized by the Newborn Metabolic Screening Act [410 ILCS 240], the Genetic and Metabolic Diseases Advisory Committee Act [410 ILCS 265], the Early Hearing Detection and Intervention Act [410 ILCS 213], and the Illinois Health Statistics Act [410 ILCS 520].DEPARTMENT OF PUBLIC HEALTH

a) Each institution or person submitting to the Department any sample for newborn screening shall be assessed a fee of $411.35.
b) Monthly Statements of fee assessment shall be mailed to facilities submitting samples for analysis.
c) Payment shall be rendered to the Department upon receipt of the monthly statement of fee assessment.
