Effective Nov 10, 200328 Ill. Reg. 17396AUTHORITY: Implementing and authorized by Section 3-12(a)(2) and (11.1) of the Liquor Control Act of 1934 [235 ILCS 5/3-12(a)(2) and (11.1)].LIQUOR CONTROL COMMISSION

a) Application forms may be obtained by writing to the Illinois Liquor Control Commission at 100 W. Randolph St., Suite 5-300, Chicago IL 60601 or at 101 W. Jefferson St., Springfield IL 62702 .
b) Application forms shall be signed and dated.
