1. a) Planning Areas:

   1. 1) The City of Chicago;
   2. 2) Cook County outside the City of Chicago;
   3. 3) DuPage, Kane, Lake, Will and McHenry Counties;
   4. 4) Municipalities with a population greater than 50,000 not located in the areas described in subsections (a)(1), (2), and (3) of this Section. Municipalities means geographic areas designated as Metropolitan Statistical Areas by the Bureau of the Census; and
   5. 5) Rural areas, i.e., all areas exclusive of subsections (a) (1), (2), (3), and (4) of this Section.

2. b) Age Groups:

   Children up to age 18.

3. c) Development Restrictions:

   No more than one children's respite care model owned and operated by a licensed skilled pediatric facility shall be located in each of the areas designated in subsection (a) of this Section. (Section 30(a)(10) of the Alternative Health Care Delivery Act [210 ILCS 3/30(a)(10)])

4. d) Bed Capacity:

   A Children's Respite Care Alternative Health Care Model shall provide care in a home-like environment that serves no more than 10 children at a time. (Section 35(3) of the Alternative Health Care Delivery Act [210 ILCS 3/35(3)]) Bed capacity within a children's respite care alternative health care model shall not exceed 10 beds and shall be inventoried as a separate category of service.

5. e) Occupancy Targets:

   Beds should have an occupancy of 40% or higher.

6. f) Need Determination:

   1. 1) One in the City of Chicago.
   2. 2) One in Cook County outside the City of Chicago.
   3. 3) Two in DuPage, Kane, Lake, McHenry and Will counties.
   4. 4) Two in municipalities (as defined in subsection (a)(4)) not located in areas specified in subsection (f)(1), (2) or (3) above.
   5. 5) Two in rural areas (as defined in subsection (a)(5)) not located in areas specified in subsection (f)(1), (2), (3) or (4) above. (Section 30(a)(10) of the Alternative Health Care Delivery Act [210 ILCS 3/30(a)(10)])

   There shall be no more than a total of eight Children's Respite Care Alternative Health Care Models in the demonstration program, located as follows:
