1. a) The Department requires the following facilities to report patient cancer incident information:

   1. 1) Hospitals;
   2. 2) Hospital-affiliated and free standing or independent laboratories;
   3. 3) Ambulatory Surgical Treatment Centers;
   4. 4) Independent Radiation Therapy Centers;
   5. 5) Independent and reference pathology laboratories;
   6. 6) Nursing homes;
   7. 7) Physicians' offices; and
   8. 8) Other Illinois facilities diagnosing and treating cancer.

2. b) The Department requests, but does not require, the following facilities to report cancer incidence information concerning present or past residents of Illinois:

   1. 1) Hospitals, clinical laboratories, ambulatory surgical treatment centers or clinics maintained by the federal government or agencies within the United States; and
   2. 2) Hospitals, clinical laboratories, ambulatory surgical treatment centers or clinics maintained by other states within the United States.
