a) Each hospice program shall have written policies and procedures that support, enhance and protect the human, civil, constitutional and statutory rights of all patients. Rights shall include but not be limited to:

1. 1) The right to informed consent that specifies the type of care and services that will be provided in the hospice program.
2. 2) The right to information regarding diagnosis and prognosis and any change in either.
3. 3) The right to review and participate in his or her plan of care.
4. 4) The right to privacy.
