- a) As a condition of issuance or renewal of a license to operate a hospice program, the applicant shall file a statement of ownership.
- b) The licensee shall notify the Department of any change in the information required in the statement of ownership within 30 days before the change.
- c) The statement of ownership shall include the following: name, address, telephone number, occupation or business activity, and percent of direct or indirect financial interest of any person having a direct or indirect interest of five percent or more in the legal entity designated as the operator/licensee of the hospice program.
