b) The quality improvement plan shall be written and shall contain, at a minimum:

In all cases, utilization management for treatment services shall be conducted by persons who meet the qualifications specified in Section 2060.320(a) or (b) for DUI evaluation or designated program services. Utilization management shall be conducted at least quarterly in accordance with the following:
