1. a) An advanced practice registered nurse granted Full Practice Authority under 225 ILCS 65/65-43 who prescribes Schedule II narcotic drugs, such as opioids, shall establish a consulting relationship with a physician and shall record that relationship in the ILPMP website (www.ilpmp.org). The advanced practice registered nurse and the physician are both responsible for entering in the ILPMP the name, DEA number, and license number of the consulting physician.

2. b) Information necessary for the ILPMP form:

   1. 1) Name (First, MI, Last);
   2. 2) DEA number;
   3. 3) Profession of the advanced practice registered nurse; and
   4. 4) Professional license numbers.

   5. c) For Schedule II Narcotics (opioids) only, the consulting physician must provide:

      1. 1) Name (first, MI, last);
      2. 2) DEA number;
      3. 3) Profession;
      4. 4) Practitioner's professional license numbers; and
   6. 5) List of specific Schedule II narcotic drugs by name.
