Application for Training and Certification
Effective Mar 26, 199923 Ill. Reg. 4278AUTHORITY: Implementing Sections 3, 4, and 5 and authorized by Section 4 of the Child Vision and Hearing Test Act [410 ILCS 205].DEPARTMENT OF PUBLIC HEALTH
Applicants for training and certification shall complete and submit, to the Department, the Application for Training and Certification Form.
