1. a) Be a citizen or lawful permanent resident of the United States;

2. b) Be a resident of Illinois;

   1. c) Be a licensed psychiatric physician in Illinois;
   2. d) Practice full time in Illinois as a psychiatric physician;

3. e) Be currently repaying educational loans;

   1. f) Agree to practice as a psychiatric physician in a designated shortage area for a specified period of time, but not less than three years;
4. g) Agree to accept medical payments; and
5. h) Agree to serve targeted populations. (Section 15(7) of the Act)
