b) Contracts with governmental agencies must contain:

1. 1) Name of governmental agency.
2. 2) Name of contracting agency.
3. 3) Description of services.
4. 4) Maximum or estimated amount to be paid, where applicable. (See Section 290.1203(e)).
5. 5) The contract term where applicable.
6. 6) Payment rates, where applicable.
7. 7) Execution date. (See Section 290.1203(c)).
8. 8) Signatures of authorized agency representatives.
9. 9) Such other provisions as may be specifically required by law.
10. 10) Any other information deemed necessary or advisable by the agency or the Attorney General.
