1. a) Application Fees

   1. 1) The fee for application for a certificate of registration as a wholesale drug distributor is $200.
   2. 2) The fee for application for a certificate of registration as a third-party logistics provider is $200.

2. b) Renewal Fees

   1. 1) The fee for the renewal of a certificate of registration shall be $200 per year.
   2. 2) The fee for renewal for a certificate of registration as a third-party logistics provider is $200.

3. c) General Fees

   1. 1) The fee for change of ownership of a wholesale drug distributor certificate of registration is $200.
   2. 2) The fee for change of ownership of a third-party logistics provider certificate of registration is $200.
   3. 3) The fee for the change of designated representative is $50.
   4. 4) The fee for change of location is $100.
   5. 5) The fee for a facility or business name change is $100.

The following fees shall be paid to the Department for the administration of the Act and are not refundable:
