1. a) Proof of successful completion of the examination approved by the Division specified in Section 1246.20 of this Part; and
2. b) The required fee set forth in Section 1246.40.

Applicants for licensure based on examination shall submit to the Division a properly completed application on forms provided by the Division, along with the following:
