1. a) The applicant's current barber or barber teacher license number;

   1. b) An official transcript showing successful completion of the supplemental cosmetology course (see Section 1175.532);
   2. c) Proof of passage of the cosmetology examination; and
   3. d) The required fee set forth in Section 1175.100.

A licensed barber or barber teacher may apply for licensure as a cosmetologist. Each applicant shall file an application on forms provided by the Division that includes:
