1. a) The applicant's current cosmetologist or cosmetology teacher license number;

   1. b) An official transcript showing successful completion of the supplemental barber course (see Section 1175.332);
   2. c) Proof of passage of the barber examination; and
   3. d) The required fee set forth in Section 1175.100.

A licensed cosmetologist or cosmetology teacher may apply for licensure as a barber. Each applicant shall file an application on forms provided by the Division that includes:
