1. a) Applicants shall submit, along with the application, the following fee for each examination.

   General Surface Supervisor	$50
   Independent Contractor Supervisor	$50
   Shaft-Slope Supervisor	$50

   Surface Mine Supervisor	$50
   Shot Firer	$50
   First Class	$50
   Shaft-Slope Worker	$50
   Crane Hoist Operator	$50
   Mine Electrician	$50

2. b) The fee for a temporary Certificate of Competency is $50.
3. c) The fee for annual renewal of Mine Electrician certification is $25.

The following fees shall be paid to the Department for administration of certificate examinations and are non-refundable.
