1. a) Fire Equipment Distributor License Fees (three-year license cycle)

2. 1) Initial License:

   1. A) Class A $900
   2. B) Class B $1500
   3. C) Class C $2100
   4. 2) Renewal of License (before expiration date or within 60 calendar days after expiration date): cost of initial license for that class

   5. 3) License Reinstatement:

      1. A) Class A $50
      2. B) Class B $100
      3. C) Class C $150
   6. 4) Renewal of License with Reinstatement (between 61 calendar days after expiration date and 1 year after expiration date): cost of initial license for that class plus cost of reinstatement for that class
   7. 5) Additional Class: cost of initial license for that class

3. b) Fire Equipment Employee License Fees (three-year license cycle), per class:

   1. 1) Initial License: $300
   2. 2) Renewal of License (before expiration date or within 60 calendar days after expiration date): $300
   3. 3) License Reinstatement: $50
   4. 4) Renewal of License with Reinstatement (between 61 calendar days after expiration date and 1 year after expiration date): $350
   5. 5) Additional Class: $300

4. c) Other Fees

   1. 1) Trainee Registration Fee: $25
   2. 2) A fee of $50 is required if a check or other order is returned by a financial institution twice because of insufficient funds.
5. d) All fees paid under the Act are non-refundable except that the Office shall refund accidental overpayment of fees. [225 ILCS 217/60(d)]
