1. a) If a corporation submits an application, one of the following persons must sign the application:

   1. 1) A principal executive officer of at least the level of vice president; or
   2. 2) An authorized agent of the corporation, if the agent is responsible for the overall operation of the wastewater source.
2. b) If a partnership or sole proprietorship submits an application, a general partner or the proprietor must sign the application.

3. c) If a publicly owned industrial user submits an application, one of the following persons must sign the application:

   1. 1) The principal executive officer;
   2. 2) The ranking elected official; or
   3. 3) An authorized employee.
