1. a) A waste volume summary which includes:

   1. 1) Total amount of solid waste accepted at the facility;
   2. 2) Remaining capacity in each unit; and
   3. 3) A copy of all identification reports required under 35 Ill. Adm. Code 811.404.

2. b) All raw monitoring data collected at the facility from the leachate collection system, groundwater monitoring network, and gas monitoring system, and in addition shall include:

   1. 1) Graphical results of monitoring efforts;
   2. 2) Statistical summaries and analysis of trends in the collected data;
   3. 3) Changes to the monitoring program; and
   4. 4) Discussion of error analysis, detection limits, observed trends.

3. c) Proposed activities for the year

   1. 1) Amount of waste expected in the next year;
   2. 2) Structures to be built within the next year; and
   3. 3) New monitoring stations to be installed within the next year.
4. d) A summary of all modifications including significant modifications made to the operations during the course of the year.
5. e) Signature of the operator or duly authorized agent.
