1. a) The requirements of Sections 742.300 and 742.305 are met;
2. b) The corrective action measures have been completed to remove any free product to the maximum extent practicable;
3. c) The source of the release is not located within the minimum or designated maximum setback zone or within a regulated recharge area of a potable water supply well;
4. d) As demonstrated in accordance with Section 742.1015, for any area within the measured and modeled extent of groundwater contamination above what would otherwise be the applicable Tier 1 groundwater remediation objectives, an ordinance adopted by a unit of local government is in place that effectively prohibits the installation of potable water supply wells (and the use of such wells);
5. e) As demonstrated using Equation R26, in Appendix C, Table C, in accordance with Section 742.810, the concentration of any contaminant of concern in groundwater within the minimum or designated maximum setback zone of an existing potable water supply well will meet the applicable Tier 1 groundwater remediation objective; and
6. f) As demonstrated using Equation R26, in Appendix C, Table C, in accordance with Section 742.810, the concentration of any contaminant of concern in groundwater discharging into a surface water will meet the applicable surface water quality standard under 35 Ill. Adm. Code 302.

The groundwater ingestion exposure route may be excluded from consideration if:
