Ill. Admin. Code tit. 35, § 1450.201 – Monthly PIMW Report

a) The Monthly PIMW Report shall be submitted to the Agency on a form provided by the Agency.
b) Entries on the Monthly PIMW Report shall be completed within 10 calendar days after the end of each month.
c) The Monthly PIMW Reports covered by the quarter shall be submitted with the Quarterly PIMW Report to the Agency.
d) The Monthly PIMW Report shall include, but not be limited to, the following information on a daily basis:
1. 1) the month and year received;
2. 2) the PIMW transport company name;
3. 3) the permitted medical waste hauler number;
4. 4) the designated facility name and location (city/state);
5. 5) the date PIMW was received;
6. 6) the total number of PIMW manifests used;
7. 7) the net weight in pounds of PIMW subject to the PIMW transporter fee;
8. 8) the grand totals of subsections (d)(6) and (7) of this Section;
9. 9) the certification according to Section 1450.104; and
10. 10) the authorized name, signature, date and telephone number.
