1. a) the name, address, officers and contact information for the broker;

2. b) the radioactive materials license numbers and issuing agency;

   1. c) the types and amounts of waste possessed or to be possessed by the broker; and (Section 4(a) of the Act)
3. d) a description of activities conducted by the broker.

All brokers shall register within 60 days after taking possession of any low-level radioactive waste. Registration shall be on a form developed by the Agency and shall include:
